This is an editorial resource on laboratory record keeping: what an entry has to contain before anyone outside the room can rely on it, how a correction is made so that it adds information rather than removing it, how a single identity is carried from the moment something arrives to the line that reports it, and where record sets usually break.
Record keeping does not fail loudly. It fails one reasonable shortcut at a time: a time written down at the end of the afternoon instead of at the moment, an identifier shortened because everybody knows which one is meant, a file saved over, a login shared because it is quicker. Every one of those is defensible on the day. Together they turn a record into a recollection, and a recollection cannot be checked by anyone who was not there.
The guide on the front page is written for small settings, where one or two people do everything and there is no quality department to fall back on. The standards it draws on were written for regulated laboratories, but the reasoning is not regulatory. It is simply what makes a record worth keeping.
The guide is written to be read once and used afterwards. It states what a given record can establish and, in the same breath, what it cannot. Where a practice has a cheaper proportionate version for a small setting, the guide says so rather than pretending that every reader has a document control department.
It is deliberately general. It does not describe, recommend, compare or make any claim about any material, item, treatment or substance, and it never tells a reader what to do with one.
